All
parents/guardians of students, K through 12, should have
received a permission slip in the mail to sign and return to the
school. This permission slip allows the school nurses to record
individual immunization and demographic data into a state data
base called CHIRP (Children and Hoosiers Immunization Registry
Program). Please sign and return it to your son or daughter’s
school as soon as possible. The county health department
requests that all students who receive the H1N1 vaccine, when it
becomes available, be registered in CHIRP. If you did not
receive the permission slip or you have misplaced it, please
print it out from our website and return.You may also fax it to
us at 765-653-3149. We are hoping the vaccine for the H1N1 flu
will be available in October and a time and place will be made
available for your child to receive it if you desire. There may
be an additional permission slip required for your child to
receive the shot at a later time.
